The San Joaquin County Home Visiting Coalition (Coalition) is a diverse group of organizations and community members committed to the well-being of children, working to carry out the goals defined in the San Joaquin County Home Visiting Strategic Plan. The Coalition’s overarching objective is to support the development of a multi-agency, sustainable, seamless, coordinated home visiting system with simplistic entry for families in San Joaquin County.

This Home Visiting Strategic Plan presents the strategic direction for San Joaquin County to coordinate, enhance, expand, and advocate for high quality early childhood home visiting programs. The Plan provides a set of five-year goals selected by the community based on local needs and strengths and home visiting best practices. It provides a roadmap of how to support families with the home visiting services they need, to maximize available funding, and to attract new funding to meet the high-unmet need for additional home visiting services.
